FEMA Flood Determination Form
The agency will allow users a three-year transition to update systems.
The Federal Emergency Management Agency (FEMA) has revised its Standard Flood Hazard Determination Form. The form number changed from FEMA Form 81-93 to FEMA Form 086-0-32. The revised form expires on May 30, 2015.
FEMA will allow users a three-year transition period ending on the expiration date so they can update systems before they must adopt the new form. Credit unions can use the previous form until that time, although they may choose the new format sooner.
The form is available at floodsmart.gov.
